Who can use these forms to change a child's name?

These forms may be used by a child's parent or conservator. However, in addition, any other person who has a legal relationship with the child must also join the petition asking the court to change the child's name. This means that both the child's mother, father and any other court-ordered conservators MUST agree to the name change. Proof of this agreement is shown by signatures of both parties on the petition. If the child is over 10 years old, then the child must also agree to the name change, in writing.

If all parties do not agree, then these forms should not be used. The child's name can still be changed, but not using these forms.
